Not Sure if We Take Your Insurance?
The doctors and our support staff would like to welcome you as a prospective new patient, and if you are already an established patient, would like to welcome you again. We look forward to serving your dental needs. Many of our established patients refer new patients, perhaps like yourself, to our office. We appreciate the kindness of their confidence and trust they have placed in us to restore, enhance, improve and maintain their dental health. We are honored to be of service to them, to you and to your family and friends.
Our group of general dentists and dental specialists is sincerely committed to providing quality dental care to you and helping our new and established patients feel good about their dental care in a pleasant and caring environment.
For your convenience, you may use our website to facilitate your registration as a new patient. Please access from our Website’s Navigation Bar “Patient Registration/Forms” There you will find two forms our office will need for you to fill out. Our new patient “Patient Registration” form. and our “Patient Medical/Dental Health History” form. 1. Download and print up the forms and bring the completed forms with you to your first appointment, or 2. Download, print up and fax the completed forms to our office. The faxing procedures will be explained in more detail in the “New Patient Information” section of our website.
Why Referrals Matter
Referrals are the life’s blood of a dental practice. According to a research firm, a typical dental office gets over 88% of its new patients through referrals. Yet, some offices are reluctant to ask for referrals for a variety of reasons. It’s always our pleasure to serve you. If you think our services were worthy of your expectations, we would greatly appreciate if you referred Michigan Avenue Dental Associates to your family and friends for their dental care. Thank you.
